Financial Policy

Physical Therapy Associates and Berkshire Physical Therapy and Wellness are committed to providing you with the best care possible. While the filing of insurance claims is a courtesy that we extend to our patients, all charges are your responsibility from the date services are rendered.

Upon your initial visit you will asked to sign a consent and authorization form. Please refer to Forms for your review.

​STANDARD INSURANCE POLICY

• ​​​​​Coverage for physical Therapy depends on your insurance company and your specific policy
• ​​​​​You may need a physician referral for physical therapy services in order to submit your claims. Please check with your carrier regarding this matter if you are unsure. Under Direct Access you may by law receive services without a physician referral, however this does not mean that your insurance
  company will pay the claim.
• ​​​​​Co-payments are due at the time of service

SELF-PAY POLICY

For patients without insurance coverage, or for those who have exceeded insurance benefits may opt to pay for services on a cash basis. Forms of payment include check, credit card, or cash. Please inquire with our office staff regarding details for payment.

***Please note that an additional fee will be charged to your account for any check returned by a bank for insufficient funds.***
