Pre-Operative Exercise for Individuals Scheduled for Total Hip Replacement
     Recent research shows strong evidence that individuals who are scheduled for a total hip arthroplasty (THA) or replacement, would benefit from a preoperative exercise program. A 4 week pre-operative exercise program improved pain and function prior to surgery as well as improved pain at 4 and 12 weeks post-surgery.
 
  
 Furthermore, an 8 week home based exercise program resulted in significant improvements in function compared to a group who received no treatment prior to surgery, at discharge after surgery and at 3 months post op.
 
By 24 months post results between the two groups were similar. This means that the pre-op exercise group had a quicker recovery of function after surgery. Another study showed that individuals who exercised pre-operatively achieved a normal walking speed 3 months before subjects who did not exercise.    
 
     Although some of the exercise programs were supervised (for research sake) a majority of the exercises can be performed safely at home. Based on the literature, it can be concluded that inclusion of pre-operative exercise therapy improves pre-operative pain and function for patients with hip osteoarthritis. When the exercise is continued post-surgery the functional improvements also continue. Consultation with a physical therapist can help individuals scheduled for a THA begin a safe and effective pre-op exercise program that an be done from the safety and comfort of your home.

Exercises for Pre-operative Hip Replacement
 
 The following are examples of exercises for individuals scheduled for a total hip replacement. Not all individuals will tolerate these exercise so consult with your physician or physical therapist before initiating any exercise program.
 
Standing Hip Abduction 
 
 Hold onto a chair as shown in picture. Raise one leg out to the side while keeping spine straight. Lower leg to floor and repeat with opposite side. Do 1 set of 8-12 repetitions progressing to 2 sets as you get stronger. Do 3-4 times a week.
Standing Hip Abduction
 
Standing Hip Extension  
 
Standing by a counter or chair for support, keep abs tight.Raise one leg slightly backward, keeping your knee straight until your foot is about 3 to 4 inches off the floor. Hold, then slowly lower. Do 1 set of 8-12 repetitions progressing to 2 sets as you get stronger. Do 3-4 times a week.
